Corinne Sicola DPT
Owner / Physical Therapist
Dr. Corinne Sicola is the owner and founder of PERfect FORMation. She has a Masters from Ithaca College and a Doctorate in Physical Therapy for Boston University.
Dr. Corinne specializes in the performing arts as she has a background in dance and gymnastics as a former performer in both.
Through her own experience and extensive training, Dr. Corinne has developed a greater understanding of the needs of her patients. Her goal is to provide each patient a better understanding of their health needs. She believes that each patient deserves personalized goal-directed care, focusing on the entire body system.
Dr. Corinne is a Certified Pilates Instructor. She is currently undergoing continued training in myofascial treatments, women’s health and visceral manipulation.


Jessica Olsakowski, DPT
Physical Therapist
Jessica graduated from the dual degree program of Seton Hall University in 2022/24 with her doctorate in Physical Therapy and Bachelor of Science in biology.
She has a strong passion for orthopedics and pediatrics but is excited to work with all types of patients. With a wide range of clinical experience in the outpatient, school-based, and subacute settings, Jessica has been exposed to numerous populations, ranging from nonverbal children with autism to geriatric patients with Parkinson’s.
She is committed to helping patients improve their mobility and quality of life through personalized, evidence-based treatments.
Outside of her professional life, Jessica enjoys reading to expand her knowledge and perspective on life. She also finds joy in staying active and challenging herself physically through running outdoors. Jessica also loves engaging in various craft projects, using creativity as a means of relaxation. However, she is happiest when playing or walking with her rescue shih tzu, Prada. Overall, Jessica is eager to learn and grow and is committed to positively impacting the lives of her patients every day. She can’t wait to meet you!
Hannah Leszcynski, DPT
Physical Therapist
Hannah Leszcynski is a licensed physical therapist who is passionate about helping individuals move better, feel stronger, and return to the activities they love. She earned both her Bachelors in Health Sciences and her Doctor of Physical Therapy degree from Saint Joseph’s University (formerly University of the Sciences) in Philadelphia.
Hannah’s background in athletics and the performing arts is what inspired her to pursue a career in physical therapy, fueling her passion for helping others move with strength, confidence, and purpose. She has experience in dance, figure skating, and baton twirling, and competed at the collegiate level as a member of the women’s track and field team, specializing in javelin, discus, hammer, shot put, and weight throw.
Her diverse background enhances her ability to connect with patients and guide them through effective, functional rehabilitation. She enjoys treating a wide variety of orthopedic conditions and is especially passionate about helping active individuals safely return to sport or recreational activity.
Outside the clinic, Hannah enjoys snowboarding, traveling, camping, hiking, reading, baking, crafting and spending time with her dog.
